Consent

I fully authorize and consent to a Senior Veterinarian online video consultation for my pet. I understand this service is online telemedicine only and has limitations compared to an in-clinic physical examination. I acknowledge that my veterinarian may recommend an in-clinic visit and that in-clinic examinations are often more effective for diagnosis and treatment. I agree to pay the listed fee, join the video session at the scheduled time, and allow the clinic to store this consent and consultation notes. I confirm I am the pet owner or authorized caregiver.
